1. Overview and Causes
 

Irregular periods refer to abnormal changes in menstrual cycle, intermenstrual period, menstrual flow, colour of menses and nature of menses. In TCM it is believed that menstrual disorder is mainly related to excess or deficiency of qi and blood as well as functions of organs and is classified into preceded menstrual cycle, delayed menstrual cycle, irregular menstrual cycle, scanty menstruation, menorrhagia (excess bleeding), etc.

2. Symptoms
 

(a) Preceded menstrual cycle: Menstruation occurs seven or more days or even ten days earlier than usual.
(b) Delayed menstrual cycle: Menstruation occurs a week or more later, or comes in forty-fifty days.
(c) Irregular menstrual cycle: Menstruation occurs over seven days earlier or later, but does so irregularly.
(d) Menorrhagia: The amount of menstrual blood is obviously more than usual but the cycle is basically normal.
(e) Scanty menstruation: The amount of menstrual blood is obviously less than usual but the cycle is basically normal.

The treatment of irregular periods by TCM is more effective than other kind of treatments.

4. Common Syndromes and Treatments in TCM

This ailment, according to TCM, has different syndromes. The treatment for these syndromes using herbal medicine and acupuncture is shown below.
 

Syndrome (a)
Irregular periods due to blood-heat

Preceded menstrual cycle or profuse menstruation which is bright or dark red, thick and sticky, accompanied with irritability and dry mouth, yellowish urine, constipation, red tongue with yellow fur, rapid pulse.

To treat by Herbal medicine
Therapeutic principle: Clearing blood -heat to regulate menstruation.

Prescription/Formula: Modified powder for clearing menstruation-heat. Moutan bark (Dan Pi) 9g, wolfberry bark (Di Gu Pi)15g, white peony root (Bai Shao)9g, dried rehmannia root (Sheng Di)9g, sweet wormwood (Qing Hao)6g, phellodendron bark (Huang Bai)3g, motherwort (Yi Mu Cao)15g, raw sanguisorba root (Sheng Di Yu)15g.
 

  

To treat by Acupuncture and Moxibustion
Therapeutic principle: Clearing blood -heat to regulate menstruation.

Prescription: Qihai (Ren 6), Sanyenjiao(Sp 6), Dadun (Liv 1),Xingjian (Liv 2), Taichong (Liv 3).

Syndrome (b)
Irregular periods due to deficiency of qi


Advanced menstruation or profuse menstruation which is thin and light in color, shortness of breath, lassitude of the extremities (hands and feet), pale tongue with whitish coating, deficient and weak pulse.

To treat by Herbal medicine
Therapeutic principle: Regulating menstruation by enhancing qi and keeping the blood flowing within the vessels.

Prescription/Formula: Modified decoction for reinforcing the middle-jiao and replenishing qi. Dangshen (Dang Shen)15g, astragalus root (Huang Qi)15g, bighead atractylodes rhizome (Bai Zhu)9g, prepared licorice root (Zhi Gan Cao)9g, Chinese angelica root (Dang Gui)9g, motherwort (Yi Mu Cao)15g, tangerine peel (Chen Pi)6g, cimicifuga rhizome (Sheng Ma)9g, bupleurum root (Chai Hu)3g.

  

To treat by Acupuncture and Moxibustion
Therapeutic principle: Regulating menstruation by enhancing qi and keeping the blood flowing within the vessels.

Prescription:
Qihai (Ren 6), Sanyenjiao(Sp 6), Zusanli (S 36), Pishu (B 20), Yinbai(Sp 1).

Syndrome (c)
Irregular periods due to blood stasis


Delayed menstrual cycle, scanty menstruation, dark-coloured menstrual blood with clots, pain in the lower abdomen, purplish red tongue, taut and unsmooth pulse.

To treat by Herbal medicine
Therapeutic principle: Promoting blood circulation and removing blood stasis to regulate menstruation.

Prescription/Formula: Modified four-ingredient decoction plus peach kernel and safflower. Peachkernel (Tao Ren)9g, safflower (Hong Hua)9g, motherwort (Yi Mu Cao)15g, rubia root (Qian Cao)12g, Chinese angelica root (Dang Gui)9g, white peony root (Bai Shao)9g, chuanxiong (Chuan Xiong)9g, dried rehmannia root (Sheng Di)9g, parched cat-tail pollen (Chao Pu Huang)6g, parched trogopterus dung (Chao Ling Zhi)9g.

  

To treat by Acupuncture and Moxibustion
Therapeutic principle: Promoting blood circulation and removing blood stasis to regulate menstruation.

Prescription:
Qihai (Ren 6), Sanyenjiao(Sp 6), Xuehai (Sp 10), Guilai(S 29).

Syndrome (d)
Irregular periods due to deficiency of kidney


Irregular menstrual cycle, profuse menstrual flow or scanty menstrual flow, thin blood with dark or light color, lack of vitality, sallow complexion, dizziness, tinnitus (ringing in ears), soreness and weakness of the loins and knees, pale tongue with white fur, thready and unsmooth pulse.
 

To treat by Herbal medicine
Therapeutic principle: Nourishing kidney to regulate menstruation.

Prescription/Formula: Modified recipe of bolus for invigorating the kidney Dodder seed (Tu Si Zi)12g, eucommia bark (Du Zhong) 9g, wolfberry fruit (Gou Qi Zi)12g, glossy privet fruit (Nu Zhen Zi)12g, eclipta (Han Lian Cao)12g, dogwood fruit (Shan Yu Rou)9g, Chinese angelica root (Dang Gui) 9g, prepared rehmannia root (Shou Di)9g, Chinese yam (Shan Yao)15g, poria (Fu Ling)8g.

  

To treat by Acupuncture and Moxibustion
Therapeutic principle: Nourishing kidney to regulate menstruation.

Prescription:
Qihai (Ren 6), Sanyenjiao(Sp 6), Shenshu(B 23), Jiaoxin (K 8), Pishu(B 20), Zusanli(S 36).

5. Healing by Other Therapies

(a) Qigong and other therapies

Generally speaking, Qigong is good for healing effect of this specific ailment, however individual results may vary. It improves the flow of the qi and the circulation of the blood, so you'll experience more energy, but, you won't see/ feel any immediate effects, as with many exercises and therapies, it takes a few weeks or even months of practice before you'll see any significant improvements.

(b) Medicated diet
Medicated diet treatment can be used as an aid to treating menstrual disorder.
1. Watermelon seed kernel:
Grind 9g of kernels of watermelon seeds into fine powder. Take it twice a day after mixing it with water. It can be used to treat preceded menstrual cycle and menorrhagia.
2. Cottonseed:
Bake 80g of cottonseeds until they become yellow, then grind them into fine powder and divide it into 14 equal portions. Take one portion in the morning and one in the evening, with brown-sugared water. It is applicable to irregular menstrual cycle.
3. Hawthorn fruit: Decoct 50g of hawthorn fruits in water and remove the residues from the decoction. Infuse the decoction with 21 to 24g of brown sugar. Take it warm. It is applicable to delayed menstrual cycle.

(c) Life style
During menstrual period, prevent from eating cold, raw or stimulating food, avoid mental stimulation and overwork.
